DE 2525XFA is for continuing a PFL care claim
DE 2525XFA is the Paid Family Leave Supplemental Claim Certification. EDD sends it with the Notice of Final Payment (DE 2525XF) for a care claim. If you still have benefits available and need to keep caring for your family member, it is the form used to request continued care benefits.
The similar form numbers can be confusing: DE 2525XF is the notice; DE 2525XFA is the supplemental certification. Check the number printed on your own document before filling it out.
Who completes the form?
You complete the claimant’s certification, and the care recipient’s licensed health professional completes the medical certification section. Return the completed form through SDI Online or the mailing instructions provided by EDD.
Before contacting the medical office, have the form in front of you and identify which section is still needed. Ask how the office returns completed paperwork, who handles follow-up questions, and when you should check back. Record the answer so that responsibility for sending the form is clear.
How to get a missing DE 2525XFA
EDD directs people who misplaced the form to request it through Ask EDD or call PFL at 1-877-238-4373. Use the Ask EDD guide to find the official written-question channel.
A useful request is: “I need the supplemental certification for my PFL care claim. I have [the final-payment notice / a missing form / an incomplete packet]. How can I obtain the DE 2525XFA for this claim?” Keep claim numbers and medical details within EDD’s official channels.
Check these details before returning it
- Claim type: confirm this concerns care for a family member, rather than bonding or your own disability claim.
- Requested period: review the dates in each completed section and resolve any mismatch with the person who completed it.
- Completion: check for missing pages, signatures, and unreadable scans.

- Proof of submission: keep a copy and any confirmation with your final-payment notice.

Does this add another eight weeks?
No. A supplemental certification requests available benefits; it does not create a new entitlement beyond the program limit. EDD describes PFL as up to eight weeks in a 12-month period for eligible workers.
