DE 2504RE requests remaining bonding benefits

DE 2504RE is the Request to Re-establish a Bonding Claim for Paid Family Leave. EDD identifies it as a way to request unused bonding benefits online or by mail. It does not give you another eight weeks.

Before choosing a form, check whether you are continuing your current bonding period or restarting after returning to full-time work. EDD gives different instructions for those situations.

Continuing leave or restarting after a break?

  • Continuing a bonding period: EDD lists DE 2504RE for requesting remaining benefits. If you have not returned to work or had a break in certification, its continuation guidance also describes certification by phone.
  • Restarting after full-time work: EDD says that returning to work full time and restarting weeks or months later requires a new claim using DE 2501F.
  • Working a reduced schedule: review the intermittent-leave and DE 2580GF guide for work reporting.

Describe your actual work and leave dates when asking EDD which procedure applies. A useful question is: “My bonding benefits ended on [date], I [did/did not] return to full-time work, and I want to request [dates]. Which form should I submit?”

Check your remaining time before submitting

EDD’s manage-benefits guidance explains that unused benefits may remain available within the applicable 12-month claim period. Bonding also has a first-year deadline tied to the child’s birth or placement. Ask EDD to confirm the dates and balance for your claim.

Keep these items together in your own records:

  • Your PFL Claim ID and the latest payment or final-payment notice.
  • A calendar of benefits already used and the additional dates requested.
  • Any return-to-work dates, hours worked, and employer pay during leave.
  • The instructions and response date on the form EDD provided.

For example, label two separate calendar blocks “already claimed” and “requesting.” This helps prevent accidentally repeating dates; it is not a calculation of EDD’s remaining benefit balance.

Submit the form through the correct claim

For supported continued-claim uploads, EDD directs you to myEDD → SDI Online → your current PFL Claim ID → PFL Upload Forms. Choose the form type, attach the completed document, and submit. If mailing, use the instructions on your form.

Save the completed copy and any submission confirmation. If DE 2504RE or its upload option is missing, use Ask EDD or the official PFL phone line to ask how to proceed for your claim.

For a first application, start with the PFL filing guide. For continued family care, see DE 2525XFA.

ConnectEDD cannot retrieve forms, submit certifications, or determine your remaining benefits.

Help with repeated calling

Want ConnectEDD to handle the redialing?

ConnectEDD handles repeated calling and requests a California PFL callback to your authorized phone number when the option is available. Callback availability and timing can vary.

You may call EDD yourself for free. ConnectEDD is a paid convenience for the repeated-calling portion and does not represent you or access your claim.
