  • Your myEDD or SDI Online access information if your question involves an online claim. Keep passwords and verification codes private.

Work, leave, and claim details

  • Your employer names, addresses, dates worked, and the wages or paid leave you expect during family leave.
  • The first day your family leave began and the claim start date you are considering or already used.
  • Your claim type: bonding, care, or military assist.
  • Your claim receipt number, Claim ID, submitted form dates, and copies of any EDD letters if you already filed.

EDD says a valid claim start date and base period cannot be changed after the claim is established, so call EDD before filing if you have questions about your start date.
