Start with the request EDD sent you
A PFL identity or eligibility-information request needs its own response. EDD may send DE 4366PFL or DE 2566 after a paper application or when it needs more information. Read the form number, requested items, response date, and return instructions together.
An initial claim attachment, account-registration check, and later EDD document request are separate steps. Completing one does not tell you whether the others are complete.
DE 4366PFL and DE 2566: identify what is missing
DE 4366PFL requests eligibility information for PFL; DE 2566 is a request for additional information. Avoid assuming every request is asking for the same identity documents.
Make a private checklist with one line per requested item. Beside each line, note the document you plan to send and whether you already submitted it. If something was sent earlier, keep the date and confirmation available for a focused follow-up question.
For paperwork accompanying a first application, use the separate PFL documents checklist.
Which identity documents does EDD accept?
EDD’s verification guidance calls for two current, unexpired identity documents: one photo ID and at least one other acceptable identity document. Examples include a driver license or passport, plus a document from EDD’s supporting-document categories. Cards should include both sides.
Use the official verification list and the particular request you received. Address, wages, Social Security number, and date-of-birth verification have different requirements.
- Open every scan and check that the corners, dates, and text are readable.
- Compare the requested period with the date on the supporting record.
- Keep the request and the exact copies you submitted in one private folder.
- Send identity documents directly through the EDD channel identified in your instructions.
Check the response deadline immediately
EDD says a DE 4366PFL response is usually due within 10 calendar days from the mailing date; follow the date on your notice. Its instructions allow you to request more time by calling PFL or mailing a request to the form’s “Return to” address.
A request for more time is not confirmation that extra time was granted. Record what EDD tells you and keep proof of your response. Check DE 2566’s own instructions rather than applying another form’s deadline to it.
If the request remains unresolved
Review your claim status and ask EDD about the specific outstanding item: “I responded to [form] on [date] using [method]. Is any part of that response still missing?”
If EDD issues a denial, read the determination and the PFL appeal guide promptly. ConnectEDD cannot verify your identity, receive your documents, or resolve an EDD account-verification request.
Help with repeated calling
Want ConnectEDD to handle the redialing?
ConnectEDD handles repeated calling and requests a California PFL callback to your authorized phone number when the option is available. Callback availability and timing can vary.
You may call EDD yourself for free. ConnectEDD is a paid convenience for the repeated-calling portion and does not represent you or access your claim.
