California PFL Documents Checklist: Bonding, Care, and Military Assist
Find which documents California PFL claims need, how to submit attachments in SDI Online, and what to check when EDD requests more information.

Submitting the application is only one part of a complete claim
The documents you need depend on whether your California PFL claim is for bonding, caregiving, or military assist. EDD says it cannot process an application that is incomplete or missing required documents.
Use this checklist to organize the paperwork, then follow the instructions on your EDD application or notice. Keep documents in your own records and submit them directly to EDD.
Match the documents to your claim type
- Bonding with a new child
- New fathers, adoptive or foster parents, and new mothers without a pregnancy DI claim generally need proof of relationship. EDD lists examples including birth or hospital birth certificates, adoption placement agreements, and foster agency documentation. Check its list for your situation. If you received pregnancy DI, follow the separate DE 2501FP transition guide.
- Caring for a seriously ill family member
- The care recipient signs Part C, and their physician or practitioner completes the Part D certification. Coordinate both parts; submitting your own portion does not tell you whether the other sections have been completed.
- Military assist
- Prepare military documentation, such as covered active-duty orders, and evidence of the qualifying event requiring your support. EDD’s preparation page describes acceptable examples for both categories.
How to upload initial claim attachments
For an online application, EDD accepts PDF, JPG, TIF, or TIFF attachments. In myEDD, open SDI Online and choose New Claim, then the bonding, care, or military-assist attachment option. Select the application’s receipt number, attach the files, and submit.
If you cannot upload, EDD directs you to the mailing address on the Confirmation Information screen. For a paper application, include the required documents in the provided envelope. These are initial-application instructions; a later request or continued-claim form may use a different submission path.
A final check before you send
- Right claim: compare the receipt number with the confirmation you saved, especially if you have both DI and PFL records.
- Readable pages: open each scan and check the edges, signatures, dates, and reverse side if it contains relevant information.
- Complete packet: make a list of each requested item and note who is responsible for supplying it.
- Submission record: keep the filename or document title, submission date, method, and any confirmation together.
These organization steps help you explain what you sent if EDD later asks a question; they do not establish that EDD accepted or approved the documents.
If EDD says something is missing
Compare the exact request with your submission record. Note the form name, requested period, and deadline rather than assuming the entire application must be sent again. Use our claim-status guide to review available information.
A focused question to EDD can be: “Your notice requests [document]. I submitted it on [date] using [method]. Is this the same item, or do you need a correction?” Share claim identifiers only through EDD’s official channels.
